Gurpreet Singh Guliani received M.D Degree in Psychiatry  from Govt Medical school and hospital in Amritsar, Punjab, India. After postgraduation he worked as a consultant psychiatrist at different locations in India. He is currently working as a Fellow in Addiction Psychiatry for the past year at University of Kansas School of Medicine, Kansas. His professional interests focus on psychopharmacology, addiction psychiatry and research work in psychiatry. His current projects include retrospective study on buprenorphine to find the mean and effective dosage in elderly. In addition he is interested in teaching medical students and is a member of numerous professional associations in US and abroad. He will looking to complete general psychiatry residency training in USA and continuing his career in united states.

Andrea Rubinstein, MD

Dr. Rubinstein is an anesthesiologist who specializes in chronic pain. Prior to medical school, she worked as a software engineer and has a special love for ‘solving problems’ and she uses an individualized problem-solving approach in all her clinical work.

Dr. Rubinstein attended Stanford University Medical School and completed her residency in anesthesiology at Duke University. While at Stanford, she became fascinated with pain and acutely aware of medicines’ limitations when it comes to solving many of the problems associated with pain.

She joined Kaiser Permanente in 2007 and served as chief of the department of pain medicine in Santa Rosa, California. She sees her role as helping patients find creative solutions to difficult pain syndromes. To do this work, she uses a multidisciplinary, bio-psycho-social approach, optimizing physical reconditioning and therapy, medications, blocks, and injections.

Dr. Rubinstein divides her time between clinical work, research in the area of opioid risk, and teaching and speaking about opioid pharmacology and novel ways to approach thinking about and treating pain.

Jeffrey Bettinger, PharmD, Pain Management Clinical Pharmacist

Dr. Jeffrey J. Bettinger, PharmD, is a Pain Management Clinical Pharmacist with Saratoga Hospital Medical Group in Saratoga, NY, and the Senior Strategy Consultant and Therapeutics Associate for Remitigate, LLC. He also recently served as an invited expert panel member for the FDA during their public workshop Morphine Milligram Equivalents: Current Applications and Knowledge Gaps, Research Opportunities, and Future Directions. He earned his PharmD from Albany College of Pharmacy and Health Sciences in 2017 with a concentration in nephrology. Following his doctoral training, he completed a PGY1 general practice residency at the Stratton VA Medical Center in Albany, NY. Immediately following his general practice residency, he trained with Drs. Jeff Fudin and Erica Wegrzyn and completed a PGY2 Pain and Palliative Care residency at the same institution.

Dr. Bettinger’s current position with Saratoga Hospital Medical Group involves working alongside primary care providers in ten different primary care clinics, where he specializes in complex and high-risk pain medication management through direct patient care and electronic consult services. He remains fortified in academia as he continues publishing in peer-reviewed journals, presenting at regional and national meetings, and teaching various pain courses at Albany College of Pharmacy and Health Sciences. He also mentors students and residents at his clinical sites of practice.

Kathleen Broglio, DNP, ANP-BC, ACHPN, CPE, FPCN, FAANP

Kathleen Broglio is an Associate Professor of Medicine at Geisel School of Medicine at Dartmouth College and  Nurse Practitioner in Palliative Medicine, atDartmouth Hitchcock Medical Center.  Her studies have included:  prevalence of opioid risk in oncology; cannabis use in palliative medicine; implementation of opioid management guidelines in palliative care; and interprofessional education in palliative and hospice. Kathleen’s clinical care focuses on complex pain management and treatment of substance use disorder in patients with serious illness.  She mentors colleagues locally, regionally, and nationally on the treatment of pain in the setting of risks for substance use disorder. Her experience includes acute and chronic pain management; and acute, ambulatory, and home-based palliative care. She completed her Doctorate in Nursing Practice at New York University, a Palliative Care Fellowship at Beth Israel Medical Center, NY, a Masters in Nursing at University of Washington, Seattle, a Bachelors in Nursing at University of Hawaii and a Bachelors in Economics at University of Maryland.

Katrina Sandoval Nickels, MD

Katrina Nickels, MD, is an Addiction Medicine Fellow at the University of Kentucky Department of Psychiatry in Lexington. She trained at the University of Kentucky College of Medicine and Internal Medicine residency program and obtained her hospice and palliative board certification through the practice pathway. She has over a decade experience at Bluegrass Care Navigators as an Associate Hospice Medical Director, a community-based palliative physician in clinic, home and nursing home settings, and as an inpatient palliative care consultant in community and tertiary care settings.  She was active in Opioid Stewardship and Pain Management Committees at the state and hospital-based healthcare organization levels.  She currently serves as an officer in the KY Chapter of the American Society of Addiction Medicine.  Her clinical interests are in the intersection of palliative care with chronic pain management and addiction medicine.  She has presented at various KY state level conferences on opioid safety in pain and palliative care, as well as at national conferences on the collaboration of palliative and addiction medicine.  She initiated a multi-institutional multi-specialty telementoring educational collaborative with a regular journal club and case discussion to support clinicians in the management of patients with serious illness, complex pain, and substance use.

David Gaffney, LMSW, BCDCH, EFT-Adv, co-presenter

David Gaffney, LMSW, BCDCH, EFT-Adv graduated with a Bachelor’s degree from Hope College and a Master’s Degree in Social Work from the University of Michigan. He completed his externship in Systemic Family Therapy from the University of Calgary Medical School. Dave is additionally a Board Certified Diplomate in Clinical Hypnosis and exam-certified as an advanced Emotional Freedom Technique (EFT) practitioner. Dave has 30+ years of clinical social work experience, serving in positions ranging from line staff to Director of Behavioral Health. He currently works as Clinical Social Worker at the Aleda E Lutz VAMC in Saginaw, Michigan, providing psychotherapy, hypnotherapy, biofeedback and mindful meditation training in the Integrated Pain Clinic. He additionally leads the Saginaw Opioid Safety Initiative Panel. David is also the Director of Training at D. K. Associates, a Midwest-based behavioral healthcare training organization, where he also holds his private clinical practice. He has served on the board for the MidWest Society of Behavioral Medicine and Biofeedback. David currently serves as national VA Subject Matter Expert for Hypnosis, Biofeedback and Guided Imagery. He also serves as a VA mentor/trainer for CBT-Chronic Pain. He currently works with the VHA Office of Patient Centered Care & Cultural Transformation Integrated Health Coordination Center as the National Champion for Clinical Hypnosis and Guided Imagery.

John Hopper, MD, DFASAM, FAAP, FACP, co-presenter

John Hopper, MD, DFASAM, FAAP, FACP graduated from the Wayne State University School of Medicine and completed residency in Combined Internal Medicine and Pediatrics at the University of North Carolina, where he served as Co-Chief Resident in Pediatrics. He is triple board certified in Addiction Medicine, Internal Medicine, and Pediatrics. From 1994 to 2005 he served on the faculty at Wayne State, and was medical director for psychiatry’s Research Division on Substance Abuse. From 2005 to 2008, Dr. Hopper served as Chief Medical Officer at Brighton Hospital in Brighton, Michigan. Dr. Hopper directed the Internal Medicine Residency and Addiction Medicine Fellowship at St. Joseph Mercy Hospital Ann Arbor from 2008 to 2015. From 2015 to 2020, Dr. Hopper was a Field Representative for the Clinical Learning Environment Review of the Accreditation Council on Graduate Medical Education. He is clinically active in both internal medicine and addiction medicine through Integrated Health Associates (IHA) where his is the current Associate Program Director for the Fellowship in Addiction Medicine at Saint Joe’s Ann Arbor and Director of the Chemical Dependency Unit at Saint Mary Mercy Livonia.

Juliette Perzhinsky, MD, MSc, FACP, moderator/lead presenter

Juliette Perzhinsky, MD, MSc, FACP is a board-certified general internal medicine and board-eligible addiction medicine physician with experience in implementing curricular changes across the continuum of medical training at Central Michigan University (CMU) College of Medicine, in collaboration with other health professions training programs. She is a graduate of Wayne State University School of Medicine and completed an Internal Medicine Residency at the University of Pittsburgh Medical Center. From 2015-2018, she served as an Associate Program Director of the CMU Internal Medicine Residency Training Program in Saginaw, MI and was charged with initiatives pertaining to patient safety and helping resident physicians increase self-efficacy with chronic pain management. She was previously selected as a Gold Humanism Scholar and was also the recipient of a Picker Gold GME Challenge Grant through The Arnold P Gold Foundation, leading important curricular changes in medical education. In her clinical practice, Dr. Perzhinsky has been delivering medication for addiction treatment (MAT) to patients with opioid use disorder after receiving the DEA X waiver to prescribe buprenorphine in 2019 and manages patients with complex chronic pain. Recently, she implemented a new integrative treatment program at the Ann Arbor VA Healthcare System – Flint Community-Based Outpatient Clinic – offering acupuncture and mindfulness meditation as part of an integrated chronic pain and MAT clinic model. She additionally invites third year medical students for clinical observation experiences during their MAT community experiences in mid-Michigan. She serves as the Project Director and co-instructor of the Drug Addiction Treatment Act (DATA) MAT waiver training to students and faculty at CMU as part of the Substance Abuse and Mental Health Services Administration (SAMHSA) grant.
